پژوهشگران نوروساینس دانشگاه لیل فرانسه در نوشتاری مروری به بررسی آخرین پیشرفت های پاتوفیزیولوژی در بیماران مبتلا تصلب چندگانه (MS) پرداختند.

??? نتایج این پژوهش نشان دادند:

بخش اول

  1. بیماری تصلب چندگانه (MS) در افرادی که آسیب‌پذیری ژنتیک به عوامل و رخدادهای محیطی دارند، ظاهر می شود.
  2. در پاتوفیزولوژی تصلب‌چندگانه، سلول‌های ایمنی جانبی تحریک و از طریق فضای تخریب شده سدخون-مغز در فضای زیرعنکبوتیه‌ای وار دستگاه اعصاب مرکزی (CNS) می شوند. این سلول‌ها شامل ماکروفاژها، سلول‌های T نوع CD8+، سلول‌های T نوع CD4+، سلول‌های B، و سلول‌های پلاسما هستند.
  3. بطور معمول تباهی میلین بصورت تخریب کانونی بوده و در نوع زود هنگام عودکننده-بهبودیابنده (RRMS) مشاهده می شود. در حالی که باقی بخش‌های ماده سفید مغز طبیعی بنظر می رسند.
  4. در طول زمان، تخریب‌های ناشی از سلول‌های T و B در مغز گسترش یافته و منجر به بروز گسترده آسیب آکسونی در سطح CNS می شوند. این هم در ماده خاکستری و هم در ماده سفید مشهود است.

بخش دوم

  1. بتدریج فرآیندهای التهابی و تخریبی CNS توسط سلول‌های میکروگلیای مقیم CNS صورت می گیرد و در کنار آن ساختارهای شبه-لنفوییدی مننژ در ادامه التهاب مغزی نقش موثری دارند.
  2. علی رغم روند و زمان متفاوت شکل‌های مختلف MS، پاتوفیزیولوژی تصلب‌چندگانه در  CNS در بیماران فرم‌‌های مختلف تفاوتی ندارد.
  3. بنظر می رسد تا زمان نگارش این مقاله، اوکرلیزوماب به عنوان داروی آنتی‌بادی غیرفعال کننده سلول‌های B در درمان نوع پیشرونده اولیه MS نقش موثری داشته است.
  4. پژوهش‌ها در زمینه شناسایی نشانگر‌های زیستی MS و میلین‌سازی مجدد در این بیماری همچنان با سرعت در حال پیشرفت هستند.

 

Progress in understanding the pathophysiology of multiple sclerosis

Abstract

Multiple sclerosis (MS) arises in people who have a genetic susceptibility to environmental factors and events, which ultimately trigger the disease.

A)

It is thought that peripheral immune cells are mobilized and enter the CNS through the impaired blood–brain barrier in the subarachnoid space. It is as acute lesions show large numbers of macrophages and CD8+ T cells and, to a lesser extent, CD4+ T cells, B cells and plasma cells.

B)

Demyelination is mostly localized to focal lesions in early relapsing–remitting (RR) MS, whereas other areas of white matter appear normal. Over time, T-cell and B-cell infiltration becomes more diffuse and axonal injury more widespread. This is leading to self-perpetuating atrophy in both white and gray matter.

C)

With disease progression, inflammatory processes are predominantly driven by the action of CNS resident microglia cells.

In addition, there is evidence that meningeal lymphoid-like structures can form and contribute to late-stage inflammation.

D)

In general, however, despite dynamic changes over time in MS pathology, lesions do not appear to differ significantly in the different classic forms of MS already identified.

All treatments approved for MS management target inflammatory components of RRMS. But, the B-cell-depleting antibody ocrelizumab is the first such treatment approved recently for primary progressive (PP) MS.

Conclusion

However, recent pathological and imaging findings have prompted reconsideration of the clinical phenotypes. These MS patients proposed by Lublin’s 2013 classification, including clinical and MRI signs of activity.

And new imaging biomarkers of remyelination are now being investigated for new strategies of MS management.
