پژوهشگران دانشگاه تگزاس ایالات متحده امریکا به بررسی وضعیت غربالگری افسردگی و اختلالات خلقی در افراد فاقد بیمه ساکن ایالات متحده امریکا پرداختند.

روش پژوهش غربالگری افسردگی:

در این پژوهش گذشته نگر (ریتروسپکتیو)،از داده های 9 مرکز بهداشت در تگزاس استفاده شد. داده ها مربوط به بازه زمانی بین ژانویه 2018 تا دسامبر 2018 میلادی بودند.

ابزارهای مورد استفاده شامل پرسشنامه سلامت بیمار (PHQ-2)، پرسشنامه سلامت بیمار (PHQ-9)، معاینه وضعیت روانی (MSE)، و مصاحبه تشخیصی بر اساس ICD-10 بودند.

یافته های پژوهش غربالگری افسردگی:

  1. کاربرد ابزارهای غربالگری افسردگی در ایالات متحده امریکا در افراد فاقد بیمه بالا است (67% از مراجعان).
  2. 24 درصد از افراد فاقد بیمه تشخیص اختلال افسردگی عمیق دریافت می کنند.
  3. بیش از نیمی از افراد فاقد بیمه که تشخیص افسردگی دریافت نموده اند، اقدام خاصی برای درمان خود انجام نمی دهند.
  4. بیماران مکزیکی تبار به صورت معناداری نسبت به سایر افراد فاقد بیمه برای دارودرمانی و روان درمانی خود اقدام می کنند.

راهبردهای کارکردی غربالگری افسردگی:

  • استفاده ادواری و منظم در کل جامعه از ابزارهای مناسب غربالگری برای افسردگی و اختلالات روان پریشی راهبردی ایمن برای حفظ انسجام اجتماعی است.
  • در صورت عدم وجود بیمه، بسیاری از افراد از درمان اختلالات روانی خود اجتناب می کنند. این امر منجر به بیمار شدن جامعه و افزایش اختلالات روانی می شود.
  • وجود طرح های رفاه اجتماعی و خدمات درمانی برای همه مردم کمک بسیار زیادی برای پیگیری درمان اختلالات روانی به ویژه افسردگی می نماید.

Depression screening and treatment among uninsured populations in Primary Care

Abstract

Uninsured populations have poor treatment engagement and are less likely to receive evidence-based interventions for depression.

Objective

The objective of the current study was to retrospectively examine depression screening, diagnosis, and treatment patterns among uninsured patients in primary care.

Method:

Study sample included all patients (N = 11,803) seen in nine community-based clinics.

Key variables included:

  • depression screener and/or a depression diagnosis,
  • anti-depressant initiation,
  • behavioral health visits,
  • patient follow up measures.

Treatment patterns from the subsample of patients diagnosed with depression were analyzed by collecting the number of behavioral health visits and antidepressant use six months (180 days) following the diagnosis.

Results:

Utilization of the depression screening tool was high (67%, n = 7,935) and 24% (n = 2,789) of the patients had a diagnosis of depression.

however, more than half of the patients with a depression diagnosis did not have a recorded treatment plan (n = 1,474).

The odds of anti-depressant use and behavioral visits for Hispanic patients were significantly greater than for Non-Hispanic patients.

Conclusion:

Universal screening with brief measures in primary care is improving, however, guideline-concordant depression treatment remains elusive for uninsured populations.
