پژوهشگران دانشگاه بقیه‌الله ایران در پژوهشی که به تازگی منتشر شده است به بررسی مقایسه‌ای استفاده از ISTDP (روان‌درمانی پویشی فشرده کوتاه مدت) و رفتاردرمانی شناختی (CBT) برای درمان اختلال درد بدون توضیح پزشکی پرداختند.

 

??? روش پژوهش استفاده از ISTDP:

در این پژوهش آزمایشی بالینی، ۳۴۱ بزرگسال داوطلب دارای اختلال درد بدون توضیح پزشکی در دو گروه درمان ISTDP و یا CBT را به مدت ۱۶ جلسه در ۱۶ هفته دریافت نمودند. سنجش‌ها پیش از آغاز درمان، پس از پایان درمان، و ۳ ماه بعد از پایان مداخله صورت گرفت.

ابزارهای سنجش شامل مصاحبه مختصر عصب‌روانپزشکی، مقیاس خودسنجی درد که بصورت لیکرت ۱۱ بخشی است، مقیاس سنجش اضطراب و افسردگی بیمارستانی (HADS)، مقیاس افسردگی بک (BDI-II)، مقیاس اغراق درد (PCS)، مقیاس خودکارآمدی دردمزمن (CPSS)، و پرسشنامه سبک‌های مقابله به درد مزمن (CPCI) بودند.

 

??? نتایج پژوهش استفاده از ISTDP:

  1. هر دو گروه CBT و ISTDP پس از پایان مداخله در میل به درمان پیشرفت معناداری نشان دادند.
  2. نشانه‌های مرضی درد بدون توضیح پزشکی در هر دو گروه پس از مداخله کاهش معنادار یافت.
  3. هر دوگروه درمانی پس از مداخله در دیسترس روانشناختی، افسردگی، و تفکر فاجعه‌بار کاهش معناداری داشتند.
  4. در هر دو گروه استفاده از آرمیدگی بعنوان راهبرد مواجهه افزایش معناداری مشاهده شد.
  5. در گروه CBT افزایش معناداری در خودکارآمدی بیماران مشاهده شد که در گروه ISTDP نبود.
  6. در پیامدهای درمانی پس از خاتمه درمان و نیز پیگیری ۳ ماه بعد هیچ تفاوتی بین دو گروه CBT و ISTDP مشاهده نشد.
  7. هر دو روان‌درمانی ISTDP و CBT برای مدیریت اختلال درد بدون توضیح پزشکی مطلوب و مناسب هستند و پیامدهای درمانی مشابهی دارند.

 

راهبردهای کارکردی استفاده از ISTDP:

  • استفاده از رویکرد درمانی ISTDP کمک مناسب و مطلوبی به درمان بنیادی ساختارهای روانی  و شخصیتی می نماید.
  • کاربردهای روان درمانی ISTDP طیف وسیعی از اختلالات روانشناختی و جسمانی را در بر می گیرند.

 

 

Psychotherapy for Medically Unexplained Pain: A Randomized Clinical Trial Comparing Intensive Short-Term Dynamic Psychotherapy and Cognitive-Behavior Therapy

Abstract

Background:

The efficacy of intensive short-term dynamic psychotherapy (ISTDP) for medically unexplained pain remains open to debate because of a paucity of high-quality studies.

Objectives:

This study sought to evaluate ISTDP as a treatment for medically unexplained pain in outpatients by comparing it with the established evidence-based cognitive-behavioral therapy (CBT) in a randomized clinical trial.

Methods:

A total of 341 adults with medically unexplained pain were randomly assigned to 16 sessions of individual manualized CBT (N = 164) or ISTDP (N = 177). The groups were assessed at baseline, after 16 weeks of treatment, and at the 3-month follow-up.

The primary outcome was perceived pain assessed using the numerical pain rating scale. The secondary outcomes were psychologic distress, depression, and cognitive variables. The cognitive variables included self-efficacy, catastrophizing, and coping strategies.

Results:

In the intention-to-treat analysis, the ISTDP and CBT groups both showed improvement in the primary outcome after treatment.

Pain symptoms in both conditions were significantly reduced. Both ISTDP and CBT groups demonstrated reductions in psychologic distress, depression and catastrophic thinking, and also increases in the use of relaxation as a coping strategy.

The CBT group showed an improvement in self-efficacy that was not obtained in the ISTDP group.

However, significant differences were not observed in the primary and secondary outcomes at the 3-month follow-up compared with posttreatment. Overall, both treatments were equally effective at the 3-month follow-up.

Conclusion:

Our results suggest that ISTDP may provide an effective alternative therapy for medically unexplained somatic symptoms of pain.
