پژوهشگران دانشگاه علوم پزشکی پیتسبورگ، دانشگاه کارلو، و دانشگاه وندربیلت ایالات متحده امریکا در پژوهشی به بررسی درمان اعتیاد به نیکوتین در بیمارستان با استفاده از داده‌های آمار زیستی پرداختند.

روش:

در یک بازه زمانی  ۳٫۵ ساله ۲۱۲۲۹ هزار نفر معتاد به نیکوتین شناسایی شدند و ۳۳٫۳% از آنها داروهای درمان اعتیاد به نیکوتین را دریافت نمودند. اطلاعات بیماران با استفاده از روش‌های بیوانفورماتیک و پشتیبانی تصمیم برای پیاده سازی بهترین روش‌های درمان مورد استفاده قرار گرفتند.

نتایج:

  1. استفاده از اطلاعات بیوانفورماتیک  منجر به افزایش معنادار زمان مشاوره برای درمان اعتیاد به نیکوتین بیماران بستری در بیمارستان شد.
  2. با کاربرد سیستم‌های بیوانفورماتیک میزان استفاده از دارو  و نظم مصرف دارو برای درمان اعتیاد به نیکوتین توسط بیماران افزایش چشمگیری یافت.
  3. استفاده از سیستم‌های بیوانفورماتیک منجر به کاهش مدت بستری برای درمان اعتیاد به نیکوتین در بیمارستان می‌شود.
  4. استفاده از سیستم‌های بیوانفورماتیک در درمان اعتیاد به نیکوتین منجر به افزایش پایبندی به دارودرمانی پس ز تریک بیمارستان می‌شود.
  5. در پیگیری‌ها ۱ ماهه بعد از ترخیض از بیمارستان، درمان اعتیاد به نیکوتین با سیستم‌های بیوانفورماتیک پایداری بیشتری دارند.

راهبردهای کارکردی:

  • استفاده از بانک‌های اطلاعاتی بیماران و مراجعان نظام سلامت کمک مهمی در درمان انواع سوء مصرف مواد بویژه اعتیاد به نیکوتین است.
  • سیستم‌های پشتیبان تصمیم و بیوانفورماتیک نقش مهمی در بهینه سازی فرآیندهای درمان اعتیاد به نیکوتین ایفا می‌کنند.
  • آموزش کاربردهای سیستم‌های بیوانفورماتیک به کادر درمانی منجر به بهبود نتایج فعالیت‌های درمانی و کاهش فرسودگی شغلی در  حوزه سوء مصرف مواد و درمان اعتیاد خواهد شد.

Using Bioinformatics to Treat Hospitalized Smokers: Successes and Challenges of a Tobacco Treatment Service

Background

Hospitals face increasing regulations to provide and document inpatient tobacco treatment, yet few blueprint data exist to implement a tobacco treatment service (TTS).

Methods

A hospital wide, opt-out TTS with three full-time certified counselors was developed in a large tertiary care hospital to proactively treat smokers according to Chronic Care Model principles and national treatment guidelines.

A bioinformatics platform facilitated integration into the electronic health record to meet evolving Centers for Medicare & Medicaid Services meaningful use and Joint Commission standards.

TTS counselors visited smokers at the bedside and offered counseling, recommended smoking cessation medication to be ordered by the primary clinical service and arranged for postdischarge resources.

Results

During a 3.5-year span, 21,229 smokers (31,778 admissions) were identified; TTS specialists reached 37.4% (7,943), and 33.3% (5,888) of daily smokers received a smoking cessation medication order.

Adjusted odds ratios (AORs) of receiving a chart order for smoking cessation medication during the hospital stay and at discharge were higher among patients the TTS counseled > ۳ minutes and recommended medication: inpatient AOR = ۷٫۱۵ (۹۵% confidence interval [CI] = ۶٫۵۹–۷٫۷۵); discharge AOR = ۵٫۳ (۹۵% CI = ۴٫۷۱–۵٫۹۷). As implementation progressed, TTS counseling reach and medication orders increased.

To assess smoking status ≤ ۱-month postdischarge, three methods were piloted, all of which were limited by low follow-up rates (4.5%–۲۸٫۶%).

Conclusion

The TTS counseled approximately 3,000 patients annually, with increases over time for reach and implementation. Remaining challenges include the development of strategies to engage inpatient care teams to follow TTS recommendations, and patients postdischarge to optimize postdischarge smoking cessation.
