پژوهشگران دانشگاه یاهابا ژاپن در پژوهش اخیر خود به بررسی مکانیزم های زیربنایی دخیل در فرآیندهای پوکی استخوان بیماران پارکینسون پرداختند.
روش پژوهش:
در این پژوهش برای بررسی پوکی استخوان (استئوپورز) 50 بیمار پارکینسون و 50 فرد همتا دارای بیماری قلبی-عروقی داوطلبانه شرکت نمودند. شاخص های مورد ارزیابی، شامل شاخص توده بدنی، مقیا رانکین، مقیاس MDS-UPDRS، و مقیاس هوهن و یار بودند. میزان پوکی استخوان (استئوپورز) بر اساس چگالی مواد معدنی استخوان (BMD) به روش DEXA محاسبه شد.
یافته های پژوهش:
- طبق بررسی های صورت گرفته، چگالی استخوان افراد دارای پارکینسون بسیار کمتر از افراد دارای بیماری های قلبی است.
- بیماران پارکینسون نسبت به بیماران قلبی/عروقی سطح بالاتری از TRACP را دارند.
- بیماران پارکینسون نبست به بیماران قلبی/عروقی سطح پایین تری از ویتامین D فعال را دارند.
- پوکی استخوان در بیماران پارکینسون زن، و دارای BMI پایین تر، بیشتر است.
- در بیماران پارکینسون دارای پوکی استخوان، سطح TRACP، و نوع 1 کلاژن متصل به N-تلوپپتید بیشتر از سایر افراد است.
راهبردهای کارکردی:
- به نظر می رسد بیماران مبتلا به پارکینسون احتمال بیشتری برای کمبود ویتامین D و پوکی استخوان دارند.
- پوکی استخوان منجر به تشدید بیماری پارکینسون در بیماران مبتلا به آن می شود.
- لازم است در پروتکل های درمان بیماری نورودژنراتیو پارکینسون درمانهای پوکی استخوان و ویتامین درمانی ادغام شوند تا از شدت بیماری کاسته شود.
Mechanism of osteoporosis in patients with Parkinson’s disease
Background:
The purpose of this study was to investigate the mechanism of osteoporosis in Parkinson’s disease (PD). Osteoporosis in PD is an important issue in maintaining the quality of life better. Because it is a risk factor for bone fracture. Osteoporosis and bone metabolism are influenced by sex and age as well as nutrition and physical activity. Therefore, we conducted these matched case-control study.
Methods:
We consecutively recruited 50 PD patients and sex- and age-matched 50 patients with chronic stage of cerebrovascular disease (CVD) as disease control showing variable level of motor sequelae.
The level of body mass index (BMI) and modified Rankin scale were matched between PD and CVD groups, which can represent nutritional status and physical activity. I
n PD group, we evaluated clinical severity using with MDS-UPDRS and Hoehn and Yahr (H&Y) stage. Osteoporosis was evaluated using with bone mineral density (BMD) by the DEXA method.
We calculated BMD young adult mean (YAM) and defined YAM<70% as an osteoporosis patient. In addition, we measured the blood bone turnover markers (BBTMs) to evaluate bone metabolism status.
Results:
PD showed lower BMD than CVD. In BBTMs, PD showed higher tartrate-resistant acid phosphatase 5b (TRACP) and lower active vitamin D (VD) than CVD. When comparing PD with osteoporosis and CVD with osteoporosis, PD with osteoporosis showed lower VD. When comparing PD with and without osteoporosis, PD with osteoporosis showed female dominancy, lower BMI, higher MDS-UPDRS part IV, and H&Y stage. Higher type 1 collagen cross-linked N-telopeptide and TRACP were observed in PD with osteoporosis.
Conclusion:
In this study, PD showed enhanced bone resorption marker TRACP and decreased VD. In addition, PD with osteoporosis showed higher disease severity. This suggests that enhanced bone resorption, VD-mediated bone remodeling failure, and disease severity are evident in PD patients with osteoporosis in addition to the well-known risks for osteoporosis.
