سایکوز درمان نشده وآسیب کارکردهای اجرایی وفراشناخت

پژوهشگران دانشگاه جاکارتا اندونزی به بررسی مدت زمان سایکوز درمان نشده و آسیب های وارده به کارکردهای اجرایی و فرا شناخت بیماران مبتلا به روان پریشی، پرداختند.

روش پژوهش سایکوز درمان نشده:

این پژوهش مقطعی عرضی در دو بیمارستان جاکارتا اندونزی انجام شد. شرکت کنندگان شامل 50 بیمار با میانگین سنی 15.9 سال بودند. ابزارهای سنجش شامل پرسشنامه اطلاعات دموگرافیک، مصاحبه تشخیصی بر اساس ICD-10، شرح حال و سابقه بیماری، و پرسشنامه سنجش رفتاری کارکردهای اجرایی- فرم والدین (BRIEF) بودند.

یافته های پژوهش سایکوز درمان نشده:

  1. در صورتی که شروع درمان سایکوز بیش از 2 ماه بوده باشد، میران تخریب کارکردهای اجرایی معنادار است.
  2. در صورتی که شروع به درمان سایکوز بیماران بیش از 2 ماه طول کشیده باشد، تنظیمات رفتاری افراد به شکل معناداری مختل می شود.
  3. در صورت فاصله بیش از 2 ماه از زمان بروز علایم و شروع درمان سایکوز، میزان تخریب های فراشناختی بیماران به شکل معناداری بالاست.

راهبردهای کارکردی پژوهش سایکوز درمان نشده:

  • تشخیص سریع و به موقع بیماری های روان پریشی، نقش بسیار مهمی در پیشگیری از آسیب های رفتاری، شناختی و فرا شناختی بیماران مبتلا دارد.
  • در حال حاضر بهترین درمان خط مقدم اختلالات روان پریشانه، دارو درمانی در ترکیب با سایکوتراپی حمایتی است.
  • آموزش های روانی و خدمات حمایت روانی به خانواده افراد مبتلا به روان پریشی در کنار گوشزدکردن خطر خودکشی فرد بیمار، نقش مهمی در کاهش خطرات دوره درمان سایکوزها دارند.

Association between duration of untreated psychosis and executive function in early-onset psychosis

Abstract

Introduction

Psychosis is a severe mental illness that causes impaired executive function (EF). The prolonged duration of untreated psychosis (DUP) is one of the negative factors in the course of psychosis.

However, the results of previous studies remain inconsistent. The aim of this study is to find out the association between DUP and all the components of EF, i.e. behavioral regulation and metacognition with all their subdomains.

Method

This was a cross-sectional study involving patients with early onset-psychosis aged 5–18 years old. DUP were collected from medical records, whereas sociodemographic data were collected by interview and EF was measured using the Behavior Rating Inventory of Executive Function-Parent Indonesian Version questionnaire.

Results

Total 50 subjects were included in the study. The median age of subjects was 15.9 ± 1.9 years old with schizophrenia as majority of diagnosis (58%). Median DUP was 2 (0; 84) months. Subjects were divided into short DUP (2 months) and long DUP (>2 months) group.

A significant association was found between long DUP (>2 months) and higher Global Executive Composite (GEC) score indicating poorer function, which consisted of Behavioral Regulation Index (BRI) and Metacognition Index (MI) (p = 0.001, p = 0.007, p = 0.001, respectively).

All subdomains of BRI and MI, except material organization, showed significant associations with DUP.

Conclusion

There was a significant association between long DUP (>2 months) and poorer EF in early-onset psychosis.
