پژوهشگران دانشگاه ملبورن و دانشگاه موناش استرالیا به بررسی وضعیت درمانی بیماران MS در پاندمی کووید-۱۹ در چگونگی ادامه درمان های تغییر دهنده سیر بیماری تصلب چندگانه پرداختند.

روش:

در این پژوهش ۱۷۸ داوطلب دارای بیماری تصلب چندگانه شرکت نمودند. ۷۰ نفر اوکرلیزوماب یا ریتوکسیماب و باقی افراد ناتالی زوماب مصرف می کردند. هیچ از شرکت کنندگان حین پژوهش مبتلا به کووید-۱۹ نشده بودند. میانگین سنی شرکت کنندگان ۴۰٫۶ سال بود.

سوالات پرسشنامه شامل دیدگاه‌های بیماران MS در پاندمی کووید-۱۹ در رابطه با تصلب چندگانه، شیوه درمانی MS، و کووید-۱۹ بودند.

همچنین اطلاعات شامل فنوتیپ بیماری، مدت زمان بیمار، نوع بیماری، مدت زمان درمان و آخرین نمره EDSS (مقیاس وضعیت ناتوانی) بیماران نیز از پرونده آنها استخراج شد.

یافته‌ها:

  1. اکثریت بیماران MS در پاندمی کووید-۱۹ نگرانی اندکی در مورد وضعیت درمانی بیماری خود و کووید-۱۹ داشتند.
  2. میزان نگرانی بیماران مبتلا به اسکلروز مالتیپل نسبت به کرونا بیش از بیماری MS و روند درمانی آن بود.
  3. سطح نگرانی بیماران مالتیپل اسکلروزیس در مورد عود مجدد بیماری و یا مبتلا شدن به کووید-۱۹ به یک اندازه بود.
  4. بیش از نیمی از بیماران مبتلا به مالتیپل اسکلروسیس بسیار بیش از سطح عمومی توصیه شده توسط دولت، به رعایت پروتکل های بهداشتی اقدام کرده بودند.
  5. تصمیم‌گیری در مورد رعایت پروتکل‌های بهداشتی در بیماران MS بر اساس راهنمایی‌ها و آموزش‌های منتشره توسط دولت در رسانه‌ها بوده است.
  6. بیماران دارای MS، علی رغم وجود پاندمی کووید-۱۹، مایلند برای کنترل بیماری خود و جلوگیری از تشدید / عود مجدد MS با رعایت پروتکل‌های بهداشتی به درمان‌های خود ادامه دهند.

راهبردهای کارکردی:

  • علی رغم اینکه میزان تشدید کووید-۱۹ در بیماران تصلب چندگانه، حدود ۴۰% بیش از افراد غیر مبتلا است، عوارض بیماری در حدی از اهمیت است که بیماران حاضرند این ریسک را پذیرفته و برای ادامه درمان خود به مراکز درمانی مراجعه نمایند.
  • افراد بیمار MS در پاندمی کووید-۱۹ به میزان مطلوبی از خود در برابر ویروس کرونا محافظت می نمایند و سطح رعایت آنها از متوسط جامعه بسیار بیشتر است.
  • ایجاد امکانات خدماتی حمایتی و درمانی از راه دور و مبتنی بر فضای مجازی برای بیماران MS به کاهش ریسک ابتلای آنها به کووید-۱۹ کمک شایانی خواهد کرد.

 Evaluating the perspective of patients with MS and related conditions on their DMT in relation to the COVID-19 pandemic in one MS centre in Australia

Abstract

Objective:

Patients with Multiple Sclerosis (MS) and on disease modifying therapies (DMTs) that can be immunosuppressive or immunomodulatory form a special group where risk of continuation of DMT needs to be taken into account with risk of contracting Covid-19. This concept can pose a degree of anxiety for patients as well as neurologists.

We aimed to evaluate patient perspectives regarding the use of Natalizumab and anti-CD20 therapies (Rituximab and Ocrelizumab) in the context of the COVID-19 pandemic.

Methods:

cross-sectional study conducted via voluntary survey filled in by patients with MS and related disorders receiving their infusional treatment in one MS centre in Australia, exploring their concerns regarding their therapy, their therapy and COVID-19, precautions undertaken in response to the pandemic, and factors impacting their decision-making.

Results:

1)

170 patients completed the survey. Of patients on Natalizumab, the majority had either no or mild concern regarding their DMT and COVID-19, and of patients on B-cell depleting therapies, again, the majority had no or mild concern, though a slightly higher proportion had a moderate level of concern.

Asked to delineate their concerns, an increased risk of contracting COVID-19 was more commonly conveyed than MS-specific factors or poor outcomes pertaining to COVID-19 if contracted, by patients in both groups.

Conversely, being invited to specifically consider the possibility of contracting COVID-19 or experience a relapse of MS, almost half of the cohort rated both of equal of concern.

More than half of the cohort were self-isolating more stringently than general government advice and government-related resources. When we followed by information provided by patient’s neurologist where the commonest means of information to guide decision making.

Conclusions:

A large proportion of patients had some concern regarding the impact of their DMT on COVID-19. whether on their risk of contracting COVID-19 or a theoretical risk for more severe disease.

the overall level of concern in most cases was at most mild. Patients on B-cell depleting therapies were more inclined to express a higher level of concern.

A similar concern was ascribed to a risk of a relapse or worsening MS symptoms compared to the risk of contracting COVID-19.

Such attitudes may underscore a willingness of patients to continue their DMT where benefits outweigh risks during future phases of the COVID-19 pandemic.
