پژوهشگران دانشگاه لوون بلژیک، دانشگاه دلفت، و دانشگاه ماستریخت هلند به بررسی امکان ترکیب داده های نواری مغزی (QEEG) و fMRI برای تولید نقشه مغزی حملات صرع در بیماران مبتلا پرداختند.

روش پژوهش:

  • در این پژوهش تحلیل ثانوی، داده های 12 بیمار مبتلا به صرع مورد استفاده قرار گرفت. ابزارهای مورد استفاده شامل fMRI با شدت میدان 3 تسلا و 32 کانال، و EEG بر اساس سیستم 10-20 با رفرنس Cz و استفاده از کلاه های سازگار با MR و نمونه گیری 5 کیلوهرتز بودند.
  • نیمرخ های مغزی افراد بر اساس فضا-زمان-فرکانس برای هر فرد به شکل جداگانه و نیز گروهی محاسبه شدند.

یافته ها:

  • ترکیب داده های نوار مغزی (EEG) و fMRI می تواند به شکلی کاملاً دقیق محل بروز حملات صرع در را هر فرد نشان دهد.
  • یافتن نقاطی که بیشترین تفاوت از نظر فعالیت عروق مغزی را دارند، می تواند نقاط کامل کننده یک حمله تمام عیار صرعی را مشخص نمایند.
  • ترکیب همزمان عملکرد مغز در EEG و fMRI در حالت استراحت می تواند نشان دهنده نقاط احتمالی بروز حمله در افراد دارای صرع باشد.

راهبردهای کارکردی:

  • استفاده همزمان از fMRI و EEG بیماران مبتلا به صرع، برای شناسایی کانون های شروع و کانون های تقویت حملات صرع در مغز این بیماران روشی نوین و با دقت بسیار بالا (بیش از 83%) است.
  • یافتن نقاط شروع و نیز تقویت حمله های صرعی، امری بسیار مهم در نوروتراپی و نیز جراحی های صرع، چه لوبوتومی و چه کاشت الکترود، محسوب می شود و کمک شایانی به جراحان مغز و اعصاب برای حداکثرسازی نتایج جراحی می نماید

Augmenting interictal mapping with neurovascular coupling biomarkers by structured factorization of epileptic EEG and fMRI data

 Abstract

EEG-correlated fMRI analysis is widely used to detect regional BOLD fluctuations that are synchronized to interictal epileptic discharges, which can provide evidence for localizing the ictal onset zone.

However, the typical, asymmetrical and mass-univariate approach cannot capture the inherent, higher order structure in the EEG data, nor multivariate relations in the fMRI data, and it is nontrivial to accurately handle varying neurovascular coupling over patients and brain regions.

Method

We aim to overcome these drawbacks in a data-driven manner by means of a novel structured matrix-tensor factorization: the single-subject EEG data (represented as a third-order spectrogram tensor) and fMRI data (represented as a spatiotemporal BOLD signal matrix) are jointly decomposed into a superposition of several sources, characterized by space-time-frequency profiles.

In the shared temporal mode, Toeplitz-structured factors account for a spatially specific, neurovascular ‘bridge’ between the EEG and fMRI temporal fluctuations, capturing the hemodynamic response’s variability over brain regions.

Results

By analyzing interictal data from twelve patients, we show that the extracted source signatures provide a sensitive localization of the ictal onset zone (10/12). Moreover, complementary parts of the IOZ can be uncovered by inspecting those regions with the most deviant neurovascular coupling, as quantified by two entropy-like metrics of the hemodynamic response function waveforms (9/12).

Conclusion

Hence, this multivariate, multimodal factorization provides two useful sets of EEG-fMRI biomarkers, which can assist the presurgical evaluation of epilepsy. We make all code required to perform the computations available at https://github.com/svaneynd/structured-cmtf.
